Temporary Health Insurance - for People With Diabetes Type 2
It will take you into the online quotes. Enter your zipcode of residency.
On the next page, complete the quote request form. It is very simple and only asks for gender and age of the persons wanting a quote. No personal information is asked and no one receives any of this information, so please feel to run quotes. If you are quoting for your children only and have more than one child, make the oldest child the Applicant. You then make the other children Dependents.
If after looking at the plans and the quotes, you want to apply, then select the plan you would like and click on apply. You will be taken to the application.
The application is very simple. You are asked Yes or No questions. If you can answer "No" to these questions, you are accepted.
You are taken to the payment page to decide how you would to pay for your health insurance. You can choose credit card, mail in a check or money order or pay by automatic bankdraft withdrawal. If you choose automatic bank withdrawal, you are also instructed to mail in your first payment and a void check with a copy of your application.
Your application is now complete. You will be at a page where you can view your application, view the certificate (if you would like to see a sample certificate, please click on Certificate) and download ID cards.
