This Plan is for

People that are uninsured for at least the past six months | |

People that are currently pregnant or plan to be pregnant - pregnancy is always covered - there is no special benefit for maternity - it is covered the same as any other illness or injury and yes, I know maternity is not an injury or illness but that is how they describe the maternity benefit | |

People that have lost or are losing their employer sponsored health insurance | |

People that are uninsurable and deciding whether to go on a COBRA plan and need a less costly option | |
|
People that have lost Medicaid, Medicare or aged out of Florida KidCare | |
|
Employers that want to offer health insurance, but cannot or do not want to have participation requirement and the employer does not have to contribute | |

Employers that want to let their uninsured employees know of an option that can get on their own (Tell them to go to www.floridahealthinsurance.com) | |

Instructions for
Enrolling in Cover Florida
Florida’s Health Insurance Plan for People that are Uninsured
Applications must be received before the first of the month to be eligible for effective dates of the first of the following as the applications must processed and forwarded to United Healthcare. Your final rate is determined by United Healthcare and is the rate in effect at the time your application is determined eligible by United Healthcare. That may or may not be the date requested. Applications must be completed in full. There are no exceptions.

CoverFlorida is designed for people that are uninsured due to health issues.
If you qualify for individual health insurance, then please also check above
buttons "Individual Quotes" and "Temporary Insurance" and see what works best
for you. If you decide that CoverFlorida is best for you, then do not
apply for or enroll in another health insurance plan as that would make you
ineligible for CoverFlorida.

First Step - Determine if you are eligible - You are eligible for CoverFlorida if (if you have a question, then email me)

You are between ages 19 and 64 (not including dependent children) and | |||||
|
You have been without health insurance for at least six months (if you meet this, no need to look further) or | |||||
|
If you are a business, no health insurance has been offered for at least six months | |||||
|
You have lost a job that provided employer-sponsored health insurance or | |||||
|
Your employer has cancelled your employer group health insurance plan, your employer cancelled your group health insurance plan | |||||
|
You have exhausted or involuntarily lost your COBRA benefits or | |||||
|
You are on a HIPAA guaranteed issue plan - we have been told a person is eligible to change to CoverFlorida - do not cancel until approved | |||||
|
You have lost your health insurance due to the death of or divorce from a spouse who has provided employer-sponsored health insurance | |||||

Second Step - Check the plans and rates and select a plan
| Open “Brochure and Rates” check to be sure you have the correct rates for the month you will become effective, rates are updated here as soon as available |

| Compare the Standard and the Standard Plus Plans |
| The brochure has a side by side comparison and there are detailed plan descriptions for both plans in the attachment “Plan Summaries” |
| Check the rates |
| Select the plan you would like to have |
| Open the Application and Enrollment Forms or Employer and Employee Enrollment Forms |
| If you qualify due to exhausting or involuntarily losing COBRA, we must have COBRA expiration letter | |
| Cobra Expiration Notice- This verifies why you lost the Cobra coverage and the date that it was lost. | |
| Certificate of Credible Coverage- This lets us know whether or not you obtained a policy start date within the 63-day window and have avoided a pre-existing condition exclusion. | |
| If eligible due to aging out of Kidcare or losing Medicaid or Medicare we must have documentation | |
| Write a check payable to United Healthcare for the amount of one month’s premium | |

Pre-Existing Conditions

Enrollees that are eligible for CoverFlorida based on not having any health insurance in the past six months or more are subject to a 12 months pre-existing condition limitation. | |
|
Enrollees that are eligible for CoverFlorida due to being on COBRA or mini cobra, exhausting COBRA or cobra or mini cobra, being on a continuation plan or having become eligible due to losing their employer sponsored health insurance, if they are on CoverFlorida within 63 days of the current coverage ending, are not subject to pre-existing condition limitations. | |

Pregnancy- Is not treated as a pre-existing condition, and would be covered up to the policy limits. |
